An order request with a pro forma layout for small businesses, freelancers, traders and service providers who need to confirm an order and request advance payment. The header holds space for the company logo, the document number and the currency code. The SUPPLIER and CUSTOMER blocks contain fields for the company name, address, company ID, VAT number and representative. The issue date is entered once, and the validity date is calculated automatically thirty days later. The place of supply is filled in, and the payment method is chosen from a drop-down list: bank transfer, cash, cash on delivery or card. The goods and services table has twelve lines with quantity and unit price, and the amount of each line is calculated by a formula. The taxable amount is the sum of all lines, and VAT is calculated at the rate entered in its own cell, 20% by default. The amount due is calculated automatically, and its label shows the currency code entered in the header. There are fields for the bank name, IBAN and BIC, the amount in words and the signatures of the person who prepared and the person who received the document. Short instructions below the print area explain which fields are filled in and how each total is calculated. The form prints on a single A4 page and is offered in fifteen colour designs.
